# Ozempic Turns On the Neurons of Hunger

## The Drug Was Meant to Silence Hunger. It Does the Opposite.

Semaglutide, the active ingredient in Ozempic and Wegovy, has reshaped obesity medicine. Earlier drugs suppressed appetite and produced 2 to 5 percent body-weight loss on average. GLP-1 drugs do the same thing and produce 10 to 15 percent or more, sometimes double that. For years, the leading explanation was simple: semaglutide quiets the brain's hunger signals harder and longer than anything before it. A study published on August 4 2026 in the Proceedings of the National Academy of Sciences by researchers at Yale School of Medicine says that explanation is wrong. Semaglutide does not silence hunger neurons. It switches them on, and those neurons appear to be exactly what the drug needs to make the weight loss stick.

## When Removing the Hunger Neurons Broke the Drug

The neurons at the center of this story are called agouti-related peptide neurons, or AgRP neurons. They sit in the hypothalamus and, when active, drive the urge to eat. Every model of GLP-1 action assumed semaglutide suppressed them. That assumption had never been tested directly in a living animal during long-term treatment. The Yale team did the test. They measured AgRP neuron activity in mice receiving semaglutide and found the opposite of what everyone expected: the neurons were being activated, not quieted. Then they genetically silenced those neurons entirely. Semaglutide kept reducing how much the mice ate, but weight loss no longer held. In some trials the mice with silenced AgRP neurons ate slightly less than the control group and still could not maintain their lower weight. The drug was moving energy out of fat tissue, not just moving food away from the mouth, and the AgRP neurons were part of how that happened. Remove them, and the metabolic channel closes.

## A Competing Study Says the Opposite, and Both May Be Right

This is not a settled question. A study from Northwestern Medicine published in the Journal of Clinical Investigation in August 2025 found the reverse: in their experiments, semaglutide and tirzepatide (Mounjaro, Zepbound) actively silenced AgRP neurons. Two peer-reviewed studies, one year apart, in leading journals, reaching opposite conclusions about the same class of drugs. The gap has not been reconciled. The most likely explanations are differences in the sex of the animals studied, the diet they were fed before treatment began, the duration of treatment, and the genetic tools used to disrupt the neurons. The Yale paper was conducted exclusively in female mice, which the researchers acknowledge as a limitation. Whether the activation pattern holds in male mice or in humans is not yet known. What is clear is that the old simple story, Ozempic quiets hunger therefore you lose weight, is no longer enough to explain the evidence.
